About this document
A Dutch death certificate (overlijdensakte) is issued by the municipality (gemeente) where the person died. I translate the certificate you received from the municipality from Dutch into Ukrainian. At a difficult time, you can arrange everything by email and post.
A scan or photo of the whole document
All text legible and the entire document visible. If there is information on the back or on further pages, include those too.
Which institution it is for
So I know what the translation is needed for. Please also ask the institution whether it requires an apostille, and on which document.
How names should be spelled
If you know how names must be written in Ukrainian, for example as in a Ukrainian passport, mention it in your message. You check the spelling in the draft.
